This option has already implemented in the Store Manager for PrestaShop
You can get access to Caption control for multi -language by clicking on title of Caption.
In the pop up window you can modify caption according to selected language - see tabs on the window.

Yes, I use this feature.
What I say is that when inserting an image, all captions take the default language's title automatically.
It would be better to auto-insert every language's title to it's language caption. Because each time we insert a new picture we have to go "Edit Caption" select every language and write again the title.

put "123" as english title
put "456" as spanish title
insert an image.
the english caption will say 123
the spanish caption will also say 123

P.S. That makes sense, if you have version Prestashop 1.4
In the PrestaShop 1.5 you can't use image caption.

Please try yourself to change product name in the PrestaShop Backend.
Image caption can not affect those tags (title, ALT) :-(

The captions are used thoroughly in both the alt and the html code. Prestashop manager automaticaly puts default language title as caption, therefore despite that I have english title, the caption is already filled with Greek title. products thumbview works with ALT and Title tag like a image caption according to Store Manager for PrestaShop
But for main picture - you can see only Product Title in ALT and TITLE tag.
