I need that every product combination has its own image AND caption. I can do it manually in Store Manager, but apparently it is impossible to achieve this using CSV import.

Yes, Store Manager allows to import product combinations and their images.

As an option you can create a test product with combinations and their images. Then export this product to the .csv in order to see the required file structure.
Concerning the caption. As we know, PrestaShop 1.5.x versions use product name as a caption for product images.
