Could you clarify your request: are you talking about custom option with file?

In Store Manager, you can export the custom options for a store. When you select Import/Export ----> Import/Export Custom Options from the tool bar, the exported file (csv) does not contain the image URL of the custom option associated to a product.

It would be great to be able to add the image URL to the export so that I can see which image is associated to a custom option

There is no image for a custom option in Magento backend.
